GENERAL SUMMARY:
Dual role including maintaining a patient caseload as an MSW and being r esponsible for the assessment and implementation of a psychosocial plan of care f! or hospice and home care patients. The implementation may incl! ude end of life care issues, advance directives, and community resource arrangements. As the Clinical Liaison, responsible for evaluation of patients referred to the agency, identifying insurance coverage, planning and coordinating referrals for home care and hospice services. Maintains liaison relationships with referral sources and community to promote and provide education on agency programs and facilitate referrals. Public relations are a very important component of the position.

Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

ESSENTIAL FUNCTIONS:
Demonstrates basic knowledge regarding psychosocial issues related to providing care at end of life and community resources for patient’s newborn thru geriatric.
Performs assessment/data collection in an ongoing and systematic manner, focusing on psychosocial, self-care, education, and environmen! tal factors.
Develops specific, measurable, realistic plans, and objectives, which enhance social service delivery.
Collaborates with physician, patient/family and other assigned disciplines regarding care needs, formation of and revisions to plan of care.
Treats patients and their families with respect and dignity. Identifies and addresses psychosocial needs of patients and family.
Advocates for needs, decisions, and rights of patients and families.
Establishes and maintains positive working relationships with patients, family members, co-workers, physicians and referral sources.

Completes timely and accurate documentation as per agency policy.

Observes the patients’ physical, mental, emotional, and spiritual conditions and collaborates with team members as appropriate.
Maintains liaison relationship with facility personnel, providing information and education on the agency services.
Attends meetings and participates in commi! ttees outside the agency as appropriate for community interaction.
! Plans and conducts regular community educations seminars to promote Agency, its staff and its programs.
Implements and maintains the Agency’s community education program for physicians, outpatient facilities, nursing and retirement’s homes, community groups, and outlying hospitals.
Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
Knowledge of MS Office software.
Keyboarding skills.
Demonstrated ability to communicate effectively.
Multitasking and prioritization skills.
Social Work assessment and counseling skills.
Knowledge of state and federal programs that provide medical care and financial support.

PREFERRED KNOWLEDGE AND SKILLS:

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
Completion of an accredited MSW program! .
Marketing experience

PREFERRED EDUCATION AND EXPERIENCE:

  • Several years’ experience in medical facility, hospice and/or home care.
  • CHP-SW
LICENSURE OR REGISTRATION REQUIRED:
Current licensure as a MSW in the State of Kansas.
Healthcare Provider BLS.
Kansas State Driver’s License
Auto Insurance equal to or greater than the Kansas insurance requirements.
