DUTIES:
The Eastern Kansas Health Care System is currently recruiting for one (1) Home Based Primary Care Nurse Practitioner to work at our Dwight D. Eisenhower VA Medical Center, Leavenworth, KS. The Nurse Practitioner, Home Based Primary Care, is a registered, professional nurse practitioner (NP) who provides primary care at home to a clinically and psychosocially complex population of chronically ill or severely disabled Veterans of the Eastern Kansas Health Care System, and may support the provision of care by others to the Home Based Primary Care population, referred services such as Homemaker/Home Health Aide population, home hospice and intermittent skilled services and contract nursing home population. The Home Based Primary Care NP is responsible for assessment, planning, coordinated provision, and evaluation of individual treatment plans for patients receiving care from the Home Based Primary Care (HBPC) program, as well as the Community Health/Contract Nursing Home, Homemaker/Home Health Aide (H/HHA) Programs of EKHCS. The NP is responsible for leading the interdisciplinary HBPC team in provision of care to their individual case load of patients, and leading the interdisciplinary team in planning and provision of care to the referred home care, hospice, and contract nursing home population.

The Nurse Practitioner maintains excellent interpersonal relationships with interdisciplinary colleagues, clients and their families/significant others, and representatives from community organizations. The Nurse Practitioner collaborates with other nurse practitioners and interdisciplinary team professionals at the local and regional level to initiate improvement in care delivery and to identify and resolve problems impacting on client care.

Key responsibilities include, but are not limited to, the following:

a. Assessing the status of HBPC patients, and planning, intervening and evaluating indicated actions;

b. Engaging interdisciplinary team members in improving the treatment plans of their population of HBPC and community health patients;

c. Supporting an atmosphere where caring for clients is the utmost concern;

d. Providing and coordinating quality care to fulfill performance expectations;

e. Collaborating with the interdisciplinary team to develop, implement and evaluate goals and objectives for HBPC and the Home Care Service; and

f. Facilitating optimal orientation and clinical competence of program staff through preceptor and mentor activities.

Current and unrestricted drivers license is required.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:

Physical standards - All applicants must meet the physical standards for the position. A physical examination prior to employment may be required.

Registration - Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The RN must maintain a current, full active and unrestricted registration to continue employment with the VA.

Educational requirements can be met as follows: Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed: a) The National League for Nursing Accrediting Commission (NLNAC); or b) The Commission on Collegiate Nursing Education (CCNE). OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. OR In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration, will meet the requirement of graduation from an approved school of professional nursing.

License and Registration A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, Territory, Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. License must be c urrent, full, active and unrestricted.

Certification Nurse Practitioners must maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. Current BLS.

Preferred experience requirements:
Home health care experience.

Experience with academic medical practice in a teaching hospital.
