SUMMARY
A Home Care Licensed Practical Nurse! provides skilled nursing care in the home setting under the supervision of a Registered Nurse.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Include the following; however other duties may be assigned:
Demonstrates knowledge of and adherence to Agency policies and applicable regulations regarding home care and Licensed Vocational Nursing practice;
Demonstrates knowledge of and application of sound nursing practice;
When required, submits documentation to appropriate registered nurse for review and signature;
Provides preventive and rehabilitative nursing care services after these services have been initiated by the registered nurse.
Provides skilled nursing care in accordance with the Plan of Care/Treatment:
Accurately follows the care plan when providing services;
Accurately reports to case manager when deviations in the care plan are required;
Seeks direction from case manager or super! visor when required.
Observes and reports regarding c! hanges in the patient’s condition and the general physical and mental condition of the patient:
Demonstrates sound decision-making skills;
Reports changes in the patient’s condition to the attending physician, dentist, podiatrist or other professional persons in a timely manner;
Reports actions to the Supervisor of Nursing, or designate, in a timely manner.
Documents on-going care of patient on appropriate forms:
Documents care clearly using accepted standards of documentation;
Submits documentation to supervising registered nurse for his/her review;
Prepares clinical and other appropriate forms according to applicable regulations, standards and Agency policy.
Participates in in-service education programs and other meetings, as appropriate:
Makes suggestions regarding in-service needs;
Attends appropriate meetings on a regular basis
Participate sin the quality improve! ment program:
Completes audits accurately;
Brings process problems to the attention of supervisor;
Participates in process improvement activities.
Participates in the health and safety program;
Uses safe work practices;
Reports unsafe working conditions to supervisor.
Demonstrates a commitment to professional growth:
Demonstrates self-direct learning to meet professional development and licensure requirements;
Attends educational programs as requested
Performs other duties as assigned by the Supervisor of Nursing:
Demonstrates willingness to accept other assignments;
Effectively accomplishes assigned duties.

Required Experience
EDUCATION AND EXPERIENCE, SKILLS, CERTIFICATES, LICENSES, AND/OR REGISTRATIONS:
Education: Current Licensed Practical Nurse licensure
Experience and Skills: One year prior professional nursing experience as a Licensed Vocational Nurse
Certificates/Licenses/and/or Registrations, etc..: Current LPN license in good standing and CPR certificate.
Other Requirements: A tuberculin clearance at the time of employment and every year thereafter; A health examination within six (6) months prior to employment or within fifteen (15) days after the date of employment; Reliable automobile; Current Driver’s License; Evidence of automobile insurance; Current CPR Certificate
WORK ENVIRONMENT:
General Office, Client's home
PHYSICAL AND OTHER DEMANDS: Employee is required to hear, speak, sit, stand, and walk daily. Employee may lift up to 50 pounds
