Purpose of Job:
The Evercare Supervisor, Nurse Practitioner will provide Clinical expertise, financial acumen and personnel management to our Nurse Practitioner team to achieve individual member's goals as well as assure budgetary and efficiency standards are met. This is an opportunity to provide leadership in the execution of the Evercare treat-in-place model of care to residents of nursing homes located in Wyandotte and Johnson Counties in Kansas. The Model o! f Care is to optimize the health of aging, vulnerable and chro! nically ill individuals living fulltime in skilled nursing facilities. The Evercare Supervisor, Nurse Practitioner is responsible for managing approximately a 50% case load and direct supervisory responsibility for 4-6 Evercare mid-level practitioners (NP’s/PA’s). The Supervisor, Nurse Practitioner reports to the Medical Director.
Major Responsibilities:
Provide primary care to nursing home residents enrolled in the United Healthcare Nursing Home Plan carrying a half time member assignment and supervising the Kansas Evercare Nurse Practitioners.
Responsible for the delivery of medical care services to a pre-designated group of Evercare members in collaboration with designated physicians responsible for an appropriate caseload of Evercare members, flexible over time to meet site clinical and organizational goals and objectives.
Coordinates, supervises and is accountable for the daily activities of the clinical team of Nurse! Practitioners.
Impact of work is most often at the team level.
Collaborates with the Medical Director as a clinical resource for medical management of members
Coaches and mentor NP team to meet utilization metrics – Acute Admits/k and SNF Days/k
Performs quality reviews including Transfer Alternative Program reviews, CAT reviews and other quality improvement functions
Monitors and coaches NP team on meeting CIS standards
Serves as a resource for coding and documentation metrics and standards
Develops and implements strategies to reduce medical costs
Participates in MAP – annual performance appraisal & pay for performance evaluations/recommendations
Clinically orients new hires
Serves as clinical expert
Responsible for establishment and maintenance of effective relationships with the Evercare members, key decision-makers, facility staff and collaborating physicians.
Re! sponsible for the clinical development and mentoring of a pre-designate! d team of Evercare mid-level providers
Direct supervisory responsibilities for 4-6 mid level providers
Actively promotes the Evercare program in assigned facilities by partnering with key stakeholders (i.e.: internal sales function, provider relations, facility leader) to maintain and develop membership growth.
Requirements
Required Education & Work Experience:
Certified nurse practitioner through ANCC/AANP as adult, family and/or geriatrics (GNP preferred), Masters prepared.
APN Licensed in Kansas or able to obtain license by start date.
Unrestricted DEA per state regulations
Three-years’ experience in adult primary care, geriatrics with prior management and/or supervisory experience required.

Aptitude to assess and identify business and personnel gaps
Strong ability to multi-task and manage multiple staff members priorities
Demonstrated leadership abilities
Capability for mentoring and developing of at least 4-6 mid-level providers in a reasonable time period.
Ability to travel regionally at least 50%
