The LPN'LVN supplements the nursing care needs of the patient as provided by the RN and as specified in the plan of care. Provides services in accordance with organizational po! licies and procedures and regulations that define scope of practice.

Essential Functions Statement(s)

Prepare clinical notes documenting outcomes of interventions.

Assist the RN and/or physician in performing specialized duties related to end-of-life care.

Assist the RN in carrying out the plan of care.

Prepare equipment and materials for treatment adherring to aseptic technique as required.

Assist the patient in learning appropriate self-care techniques.

Assess patient/family response to care.

Ensure communication of information to appropriate team members.

Complete annual on-going tr! aining/contining education.

Assist with special proje! cts and other responsibilities as assigned.

Required Skills

Understand the practices and procedures relating to referral of victim of abuse/neglect/mistreatment/exploitation to the appropriate community agencies. Promotes Community education and awareness of hospice services. Recognize and maintain professional boundaries in relationship with patient/family/caregiver. Must have sensitivity to issues of loss and grief. Has the ability to deal effectively witht he demands of the job.

Required Experience

Education: Graduate of an accredited school of nursing that leads to LVN/LPN

Experience: 1-3 years of related health care experience

Computer Skills

To perform this job successfully, an individual should have knowledge of Internet software, Work Processing software, Healthcare software.

Certificates and Licenses

Current Licensed Practical Nurse or Licensed Vocational Nurse in state w! here work is performed with no restrictions.
