Responsibilities:
Responsible for the treatment of members in the nursing
home and skilled nursing facility settings. Performs
comprehensive SNF assessment to include! psychosocial
factors (when applicable) identifying the an! ticipated
short-term and long-term needs of the member.

will partner with various physicians in the building to
provide integrated care to patients referred from the
various specialties within the pavilion including Family
Practice, Pediatrics, and Ob-Gyn. Part of a

multidisciplinary team responsible for assessment,
treatment planning and treatment delivery.

Requirements:
Masters of Nursing Science (MSN) with preparation as a
Advanced Practice Nurse (NP or CNS) Two (2) years of
experience

CURRENT STATE LICENSE
