The Certified Nurses Aide is responsible for providing competent home health aide services to patient/clients with a chronic illness who do not need to be homebound and are not necessarily seen intermittently in accordance with the policies of the agency and the nursing plan of care for the patient/client.
DUTIES AND RESPONSIBILITIES
- Skillfully and safely provide personal care as outlined on the plan of care, in! cluding hygiene needs and incidental tasks that are essential to the patient/client’s health and necessary to prevent or postpone hospitalization.
- Assist families to maintain their normal and usual activities of life during a critical period of time.
- Assist in re-establishing activities of daily living encouraging as much independence as possible.
- Observe for incidental needs of the patient/client.
- Exercise initiative and judgment in performance of duties.
- Maintain charting and records as required by the agency and federal and state regulations, assuring that charting follows the plan of care established by the case manager.
- Maintain patient/client notebook; update as needed due to admissions, discharges and changes in plan of care.
Previous experience in home health preferred.
