More details about this vacancy opportunity kindly read the description below. Nurse Practitioner will work with patients as a Care Consultant. As a Care Consultant, will help patients that are in transition with their medical care. May provide ed! ucation on the disease process, and or coordinate transitioning patient into home health, skilled nursing settings.
Essential functions include, but are not limited to the following:
Responsible for the treatment of members in the nursing home and skilled nursing facility settings. Performs comprehensive SNF assessment to include psychosocial factors (when applicable) identifying the anticipated short-term and long-term needs of the member.
Identifies and refers members into appropriate disease and case management programs. Identifies the need for hospice care as appropriate in the âterminal periodâ of chronic diseases.
Refers appropriate members to case managers for hospice program election or to be designated as a âPotential Hospiceâ candidate and monitored for decline.
This provider collaborates with the physicians for all patient related responsibilities and deals directly with skilled nursing facility staff members t! o coordinate patient care requirements.

Qualifications and Requirements
The successful Nurse Practitioner or Physician Assistant candidate will have a certificate of completion from an approved program and be eligible for or certified by their respective Texas board organizations. The candidate must be certified to work with the adult population age group. Must have current BLS. Previous experience in a skilled nursing facility or with geriatric patients is preferred.
The candidate should have excellent people skills, demonstrated concern for the health and well-being of others, superior written and oral communication skills, computer literacy including electronic medical record ! documentation and the ability to work both as a team member and independently.
